Fluid Overload

Critical Urgency

Excess fluid volume causing cardiopulmonary strain — most common complication during large-volume (1L+) infusions.

Protocol Steps

  1. 1

    Stop the infusion immediately

  2. 2

    Place patient in upright position

  3. 3

    Administer oxygen if available

  4. 4

    Do not administer any more fluid

  5. 5

    Check vitals (BP, heart rate, respiratory rate, oxygen saturation if available)

  6. 6

    Auscultate lung fields for crackles/rales

  7. 7

    If rales present, notify oversight physician immediately

  8. 8

    Anticipate Lasix (furosemide) administration — do not administer without physician order

  9. 9

    If respiratory distress, hypoxia, or patient deteriorates, call 911

  10. 10

    Monitor and document vitals every 5 minutes until resolved or EMS arrives
